Preapplication Appointment Form

To schedule a Preapplication appointment: 

• Fill in all applicable sections.
• Submit your application, attachments, and payment in person at City Hall during counter hours, by mail, or electronically at planning@milwaukieoregon.gov.
• When we receive your complete application, we will schedule your appointment per the timelines on the form.
